Welcome to Children's University of Chapel Hill Admissions. To apply for an opening within our program, we will need:

Completed Application Form (download/view).
Medical Report completed by a North Carolina physician:
- Immunization Record
- Child's Medical History
A signed Discipline Policy (download/view).
A signed form stating that you have received and read a copy of the Parent Handbook/Operational Policy:
- Parent Handbook
- Receipt of Policies
Permission to Photograph Form (download/view).

These documents require Adobe Reader to view.

If you are accepted and want to guarantee your space, a $300 non-refundable payment is required. You will also have to pay your first month's tuition prior to beginning the program. Tuition rates are listed below.

Monthly Tuition Rates (2009-2010 School Year)

Child's Age	Tuition
Infants & Young Toddlers	$1,250
Older Toddlers	$1,025
2 Year Olds	$975
3-5 Year Olds	$925

These rates are in effect from September 2009 - August 2010.

Parents who participate in the county scholarship programs are required to pay their parent portion plus the difference from the county contribution. If you are using a scholarship and are uncertain about your monthly payment, call our director and she will work out what the cost will be for your child in our program.
